Eczema Consultation Agreement


This agreement is made between the eczema consultation seeker (referred to as the “Client”) and the Canada-based www.eczemaconsulting.com (referred to as the “Consultant”) for the alternative non-medication treatment methods specific to client’s eczema.


1. The consultant and the client will go through a questionnaire of 30 to 45 minutes over an online zoom meeting. The client will provide accurate and detailed information according to the questionnaire about client’s eczema condition, medical history, and any ongoing treatments.


2. The consultant will do a case analysis based on the information given by the client from the above zoom meeting. The consultant, based on consultant’s expertise, knowledge, experience and available information, will provide tailored suggestions of the alternative non-medication treatment methods specific to client’s eczema.


3. A written Eczema Consultation Suggestions will be emailed to the client within 5 work days after the above zoom meeting. The client agrees to follow the consultant’s Eczema Consultation Suggestions to the best of client’s ability.


4. The client acknowledges that the consultant’s Eczema Consultation Suggestions are not a substitute for professional medical advice and treatment. Any instruction, treatment and prescription from client’s doctor or dermatologist should be followed as well at same time.


5. Though the alternative non-medication treatment methods for eczema are effective to most people, exception exists. The client acknowledges the consultant is not liable for any outcomes, including adverse reactions and complications, resulting from the Eczema Consultation Suggestions provided.


6. The cost of this eczema consultation service is USD$150.00, payable before the online zoom meeting. Upon receiving the payment remittance, the consultant must arrange the online zoom meeting according to the client’s appointment, a zoom link must be sent to the client 24 hours before the appointed time.


7. The client may terminate this agreement by email notice at least 24 hours before the online zoom meeting. An USD$50.00 administration fee plus bank charges will be deducted, the remaining fund will be returned to the client within 5 work days. In any other condition this agreement is not cancellable.


8. The client’s privacy is strictly protected. Only the consultant has access to client’s information. The client’s all information will be deleted after 18 months from the client’s last consultation.


9. The client and consultant agree that this eczema consultation is not a professional medical advice. This consultation does not intend to replace or interrupt client’s ongoing treatments from the client’s doctor or dermatologist.


10. This agreement shall be governed only by Canada’s related law; regardless which country or area the client lives in.
